Right to Work (Passport, Birth Cert, VISA’s etc)

Proof of address x2

Proof of National Insurance

Proof of PVG

Proof of SSSC (if registered) OR NMC cert for nurses

For NHS work: Proof of immunisations (BGC, MMR, chicken pox and Men B)
